How do you do a QADRO after the divorce is final?

My ex-husbands attorney was suppose to complete the QADRO over 11 years ago during our divorce proceedings. The QADRO was not completed as he and his parents thought this was a way to keep me from not getting his pension I am entitled too. I need a QADRO done CHEAP any ideas how I can get this done? His employer will not act on getting me the pension without one. Can I sue my ex-husband and his attorney for not completing the contractual agreements made in the divorce decree? My attorney is to blame also as I she only followed up by sending letters to his attorney requesting the QADRO and did nothing futher. This has been going on for going on 12 years, the money is for our daughter for college not for me his ex-wife. I can't get anyone to help me!!!!!!
